Visiting Nurse and Hospice for Vermont takes the confidentiality and security of our patients’ information seriously. Regrettably, this notice is to inform our patients of a recent incident that may have involved some patients’ information.

On October 8, 2019 we determined that there may have been unauthorized access to an email account which contained some patient information.  The unauthorized access occurred in July 2019.  We worked with a third-party forensic investigator and conducted a review of the documents which may have been viewed or accessed by the unauthorized actor.  Some patients’ information may have been contained in those messages, including patients’ names, dates of birth, addresses, health insurance information, clinical information related to services or care received from Visiting Nurse and Hospice for Vermont, and, in some cases, social security numbers. We have no indication that any patient’s information has been accessed or misused.

We began mailing letters to affected patients on December 6, 2019. We are offering credit monitoring and identity protection services to those patients’ whose social security number may have been contained in the messages potentially viewed or accessed by the unauthorized actor.

We have established a dedicated call center to answer questions that our patients may have.  If you believe you are affected and do not receive a letter by January 6, 2020, please call 1-844-931-1878 Monday through Friday, 9:00 a.m. to 6:30 p.m. Eastern Time. We recommend affected patients review the statements they receive from their health care providers. If they see services they did not receive, patients should contact the provider immediately.

We sincerely regret any concern or inconvenience this incident may cause our patients. To help prevent something like this from happening in the future, we are taking steps to further enhance the organization’s data security procedures.
